1. Patient must be currently uninsured if they want to apply for Medical and Dental. If the patient has medical insurance without dental coverage and meets the income criteria, we may accept the patient for dental only.

2. Must be a Duval County Resident and patient must be 18 years or older

3. Income must be below the 200% the poverty line according to current guidelines (Click here to see income limits)

 IMPORTANT INFORMATION

We only take new patient applications every weekend from Saturday 12:00AM until Sunday 11:59PM. 

This is due to limited administrative staff time to process current applications and match patients with available providers.

1. Fill out the online application form (Complete form here)

2. Upload the income/lack of income support within the online application (You must upload a file to complete the application), make sure that the paystub/statement is properly labeled, so our staff can verify that it is actually yours.

– If you work as employee: Send last 4 weeks paystubs.

– If you work independently: Send last 2 months bank statements or CashApp statements with government ID.

– If you are retired or received social security: Upload your social security statement or other proof of pension.

– If you don’t work and someone is supporting you: Send the Notarized Letter of Support (Click here to see template)

Keep in mind that you may need to wait 12-16 weeks before your first appointment.

Please note that our clinic provides PRIMARY care, both medically and dentally. If you are experiencing an emergency call 911 or go to your nearest Emergency Room. If you are experiencing something that requires to be seen sooner you can contact Healthlink JAX at 904-925-CARE(2273).

Dental Clinic Waiting Time: Approximately 12-16 weeks to schedule an appointment since all our Dentist are Volunteers. Our services are provided at no cost, including general examinations, dental x-rays, cleanings, fillings, and extractions.
