PATIENT ELIGIBILITY/OFFICE HOURS:

Monday – Friday 9:00 a.m. to 1:00 p.m. & 2:00 p.m. to 4:00 p.m.

CLINIC HOURS: Based on Provider Availability – established patients must call the clinic to schedule appointments.

*Medical and Dental Clinic hours are subject to change depending on the availability of providers and holidays.

The COVID-19 Pandemic means that Community Health Outreach serves even more people in our community who are food insecure, medically and dentally uninsured, and lack essential baby supplies such as diapers and formula. In all aspects, our numbers of clients and patients are growing and we need your donation to help us keep up with the need.
