Healing Hands – Dental and Medical Clinic

Healing Hands is the free Community Health Outreach clinic. We provide quality Emergency Dental Services, Primary Care Medical Services, as well as Mental Health Counseling Services for the uninsured residents of Duval County who meet financial criteria.

All Services are By Appointment Only:
Our quality Dental, Medical, and Mental Health Services are provided by local volunteer Dentists, Doctors, Hygienists, Nurses and a variety of other professionals.

To Make an Appointment:
All patients must complete an application and return it with the required documentation. There are two separate applications for Medical services and Dental services. If requiring both services please make sure to complete both applications and attach required documentation. Questionnaires may also be picked up in the lobby of our new clinic, located at 5126 Timuquana Road Jacksonville, FL 32210. Please call 904.573.1333, option 1, should you have any questions. Questionnaires can be mailed to us or best brought into our lobby usually open M-F 10am to 2pm. Please do not fax applications.

Starting September 1, 2015 all applicants must provide proof of income using one of these with the application: Most recent tax transcript or proof of non filing via transcript from IRS (Use form 4506-T to get transcript). Other accepted verification Food Stamps, Unemployment/Workers Compensation, 1 month pay stubs, Child Support/alimony, Social Security/disability, Veteran’s Disability, or TANF (Temporary Assistance for Needy Families)

Applicants must provide government issued photo I.D.

Eligibility Guidelines and requirements:

We provide patients with complete information about all treatments prior to delivery. A signed Patient Consent Agreement is requested prior to the receiving any procedure or treatment. We are in HIPAA Act compliance to respect and protect patient privacy regarding medical treatment, as well as the confidentiality of medical records.

  1. CHO is proud to announce a new service model which will utilize the dental services of The Sulzbacher Center at the recently constructed Healing Hands Dental and Medical Clinic.
  2. Collaboration will combine the dental resources and management expertise of The Sulzbacher Center with the dental outreach services of CHO.
    • Healing Hands clinic will continue to offer free emergency dental services on Monday’s and Friday’s utilizing volunteer dentists and Sulzbacher support staff for patients currently on the waiting list.
    • Existing patients will also be offered the option to schedule an earlier appointment on Tuesday, Wednesday, Thursday for a nominal fee of $25.00. Clinic will be fully staffed with Sulzbacher personnel.
    • Small fee will be used to offset some of the costs associated with running the clinic.
  3. 1st phase of the partnership will increase the number of appointments for emergency dental care for the estimated 600 people on the waiting list. Long term objective is to incorporate comprehensive dental services into this collaborative model.
  4. The Sontag Foundation anticipates that this innovative model will have a long term impact on addressing the service gaps in oral health care as well as increasing the awareness of the importance of good oral health care on the quality of life of individuals in our community.
    • The Sontag Foundation recently conducted an extensive 16 month review to obtain an understanding of the issues impacting the self-sufficiency and quality of life of NE Florida residents. From that research, it became evident that there was an overwhelming need to address the service gaps around oral health access in our community for underserved and vulnerable populations.
  5. The Sulzbacher Center: The Sulzbacher Center operates two Federally Qualified Health Centers, both with a level 2 Medical Home designation. (the Healing Hands Clinic will not fall under this umbrella as it is a private clinic)
    • The Sulzbacher Dental Clinics offer an array of oral health services for adults and children, such as fillings, bridges, crowns, dentures and oral surgery. In 2014, these clinics treated around 3,000 new dental patients, and provided over 6,600 dental services.
    • In addition to dental, Sulzbacher Clinic’s other services include primary care, eye care and behavioral health, ensuring a comprehensive approach to patient care. The Sulzbacher Downtown Clinic serves residents of Duval and surrounding counties who meet financial eligibility criteria.
    • The Sulzbacher Beaches Clinic serves patients who reside in 32233, 32250, 32082, 32224, 32266 zip codes who meet financial eligibility criteria

o To further address the complex health issues facing homeless persons following hospital discharge, the Sulzbacher Center added a 20-bed Medical Respite facility at the downtown campus in 2013.

The new agreement allows us to improve services!  We still accept applications and have available for free the same services as in the past.  We ask for your prayers as we transform how we deliver the services.

  • Healthcare Professional Volunteers – We need your expertise. If you are a Dentist, Hygienist, Dental Assistant, Medical Doctor, Physician Assistant, Nurse Practitioner, Nurse, Dental/Medical Technician, Dental/Medical Office Administrator wishing to give of your services. Free Dental Clinic days are normally Monday and Fridays limiting the need for volunteer Dentists to those days. Medical volunteers needed anytime.
